Any childhood cancer survivor not currently receiving active treatment, has been in remission for no more than 6 years, between the ages of 9-13, and with parental or legal guardian consent, is eligible to apply.
Legal guardians of applicants must read and consent to the terms outlined in the Name and Likeness Release. Download Release
Fill out the application and all required fields. Once the application is finished, please upload the application along with your video to the submission form.
2. Gather Materials

- Fill out child overview and relative experiences
- Share words of wisdom
- Have your parent/guardian describe the affects childhood cancer has had on your family
- Write a short essay (500-word max) on why you want to be the HHOW Ambassador
– Get a letter of recommendation from your doctor
– Create a Video (2-minute max) telling us your experience and tell us in your own voice why you want to be an ambassador.
3. Fill Out
Submission Form
All fields of the application must be filled out. You must submit a video proposal of “why” we should select you as the ambassador. Please state your first and last name at the beginning of the video.
